Case Management is now known as Community Integration Services. These services are offered to assist clients with integration into the community and accessing the resources needed to stabilize and sustain in the community.
The goal is to offer Community Integration, to assist both, adult and children (and their families), to access the resources and develop skills for increased independent living within the community.
Based on the client’s needs, services can consist of:
- Coordinating Referrals and Services with other professional
- Assist with Housing, Food, or Employment Assistance
- Increasing Client knowledge of Community Resources
- Help you finding support Groups
- Advocating or the Client’s Needs
- Providing Information, Consultation, and Problem-Solving Support
- Assisting with Access to Transportation Services
